Message From the Nurse:
All children upon entering into a school need a physical and proof of immunizations.  Physicals are required for students in kindergarten, first, third,  fourth and seventh grades.  When your child has a scheduled physical, please request a form from the school.  If your child has any injections, please request that the doctor writes the date and the name of the immunization on a paper.
